Please complete the following form as accurately as possible then submit it when ready. We will send a confirmation email to the email address provided - click on the confirmation link in the email to complete the activation. Unconfirmed activations will be purged after 1 hour.

We collect this information to help safeguard ticket holders against fraud. We don't share it with anyone. We take your privacy very seriously and have taken measures to ensure your personal information remains secure.

OUR STANDARDS

In the interest of fostering an open and welcoming environment, we as organizers pledge to making participation in our event and our community a harassment-free experience for everyone, regardless of age, gender identity and expression, ethnicity, nationality, race, religion, appearance, ability, level of experience, or sexual orientation.

Examples of behaviour that contributes to creating a positive environment include:

  • Welcoming and inclusive language.
  • Respecting viewpoints and experiences that differ from your own.
  • Gracefully being receptive to and accepting constructive criticism.
  • Focusing on what is best for the community.
  • Showing empathy towards others.
  • Helping those in need.

Examples of unacceptable behavior include:

  • The use of sexualized language and unwelcome sexual attention or advances.
  • Trolling, insulting/derogatory comments, and personal or ideological attacks.
  • Harassment of any kind, in public or private.
  • Excessive intoxication.
  • Theft or willful mistreatment of personal property.
  • Violence of any kind.

ASSUMPTION OF RISK, RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY AGREEMENT

BY AGREEING TO THE TERMS BELOW YOU WAIVE CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE

I wish to attend and participate in Harvest Festival and understand I do so at my own risk. I am aware that attending and participating in Harvest Festival has many inherent risks. Despite these and other risks, and fully understanding such risks, I wish to participate in Harvest Festival and related activities and hereby assume all risks of same.

In exchange for being permitted to participate in Harvest Festival, I hereby agree as follows:

  1. TO RELEASE all organizers, owners, employees, agents, representatives and volunteers of Harvest Festival and associated property, including Justin Martin, 2382090 ONTARIO INC., o/a Harvest Festival, Peter Camani o/a Screaming Heads / Midlothian Castle (collectively referred to as the “Released Parties”) from liability for, and I hereby waive any and all claims for, any loss, damage, injuries to person or property, death, claims, demands, lawsuits, expenses and any other liability of any kind, that I may suffer or my next of kin may suffer directly or indirectly arising out of or in connection with my participation in or attendance at Harvest Festival due to any cause whatsoever, INCLUDING NEGLIGENCE ON THE PART OF THE RELEASEES.
  2. TO HOLD HARMLESS AND INDEMNIFY the Released Parties from and against any and all accident, loss, damage, injury to person or property, or death, sustained by me or others, and any other liability, claim, action, cost and expense (including attorney’s fees and expenses), arising out of, connected with, or resulting from my attendance and participation in Harvest Festival and/or related activities on Midlothian Castle and/or the property at 981 Midlothian Road, Ryerson, Ontario.
  3. That this release of liability, waiver of claims and indemnity agreement shall be effective and binding upon my heirs, next of kin, executors, administrators, successors and assigns, in the event of my death.

By clicking "I AGREE", I acknowledge that I have read and understood the above and that I freely agree to the above assumption of risk, release of liability, waiver of claims and indemnity agreement.

COVID-19

Harvest Festival will develop, implemented and follow COVID-19 protocols to reduce the spread of the COVID-19 as maybe required by applicable regulation at the time of the event in September.

Nonetheless, it is the responsibility of all individuals attending Harvest Festival to protect the health and wellbeing of the community by adopting their own personal health and safety practices in accordance with public health guidance at the time of the event. Harvest Festival strongly encourages individuals to adhere to applicable public health guidelines and Harvest Festivals’ personal responsibility guide as outlined below.

  • Do not come to Harvest Festival if you are feeling unwell or are experiencing the symptoms of COVID-19, you have been in contact with someone displaying COVID-19 symptoms, you have been in direct contact with a confirmed case of COVID-19, or you have traveled outside of Canada within the last 14 days.

In exchange for being permitted to participate in Harvest Festival:

  1. I acknowledge the highly contagious nature of COVID-19 and I voluntarily and knowingly assume the risk that I (or any person legally in my care) could be exposed or infected by COVID-19 as a result of attending Harvest Festival.
  2. I agree on my own behalf (and on behalf of any person legally in my care) that by clicking "I AGREE" below, I am voluntarily giving up any right to sue Harvest Festival and all organizers, owners, employees, agents, representatives and volunteers thereof and associated property, including Justin Martin, 2382090 ONTARIO INC., o/a Harvest Festival, Peter Camani o/a Screaming Heads / Midlothian Castle in the event that I (or any person legally in my care) is exposed or infected by COVID-19 as a result of attending Harvest Festival.

By clicking "I AGREE", I acknowledge that I have read and understood the above and that I freely agree to the above assumption of risk, release of liability, waiver of claims and indemnity agreement.
